Social Worker- (Behavioral Health Interdisciplinary Program)
To provide clinical social work services as part of an interdisciplinary team in the Mental Health Service.
Services include provision of case management, psychotherapy, advocacy, crisis intervention and coordination of linkages to other VA or community service providers.
This is accomplished in collaboration with other members of the team or other services as indicated.
The work is primarily outpatient but could include inpatient services as needed.
Complete psychosocial assessments of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others.
Treatment planning/goal setting.
Collaborate with other service providers in reassessing the Veteran's needs.
Maintaining and using a current social work resource file of VA and community services and programs and enhancing the content to the benefit of Veterans.
Collaborates and consults with other interdisciplinary treatment teams and programs to promote and enhance the care of the Veterans.
Acts as advocate when it serves the best interest of the Veteran and family.
