Social Worker-Dementia Care Coordinator

The position is responsible for assessing, planning, organizing, and coordinating all Dementia Care Services and programs at the VA IIliana Health Care System (VAIHCS) including inpatient Community Living Center, outpatient Geriatric Clinic, and the Dementia Committee. The incumbent also provides clinical Social Work services for patients in the Community Living Center and outpatients with diagnosis of Dementia and their caregivers. Also serves as a member of the Geriatric Clinic Team and the Dementia Committee. The incumbent is also the admissions liaison for all Geriatric and Rehabilitation Service referrals. Assumes responsibility for ensuring that all Dementia programs and services at VAIHCS comply with VHA and Medical Center policy, Federal, State and Local regulations and meet Joint Commission and Long-term Care Institute requirements. Assuring Dementia programs and services interface with other VA and community programs to provide continuity of care to patients and caregivers. Serves as the point of contact to Central Office, VISN and the community regarding Dementia Care at VAIHCS. Responsible for collecting and maintaining statistical data, preparing reports and overseeing performance improvement activities related to Dementia Care programs and services. Managing Dementia Care/ Geriatric Clinic Consults and coordinating the team's response. Coordinates program activities with key inpatient and outpatient Dementia Care staff and stakeholders to ensure continuity of care and high-quality care for patients with Dementia. Participates in CLC Compare quality monitoring and improvements and may be assigned as Champion for appropriate monitors. Assists with other Geriatric and Rehabilitation monitoring for the Community Living Center. Develops new initiatives and refines present programs and services to meet the needs of Dementia Care patients in the face of ever-changing environment, care practices and new technology. Evaluates programs and services and plans for new programs and services based on outcome measures, customer satisfaction indicators and evidence based best practices. Coordinates program activities with key Service Chiefs and inpatient and outpatient providers to ensure seamless programmatic development and provision of Dementia Care services. In conjunction with other key staff, markets the Dementia Care programs and services to VAIHCS staff, patients/families and the community. Completes psychosocial assessments, develops treatment plans and provides counseling for patients/families referred to the Dementia Care Consult Team and Geriatric Clinic. Provides other Social Work interventions including but not limited to crisis intervention, advocacy and linkage to community resources. Incumbent provides individual, group and family treatment interventions as indicated to assist patients, family members and significant others coping and dealing with loss and grief experiences in relation to dementia and terminal illness. Assists with coverage of general social work coverage of CLC and other areas when needed. Develops and delivers programs and materials to Medical Center staff and patients/families regarding Dementia Care programs and services. Develops and delivers educational programs to staff and patients/families regarding clinical aspects of Dementia Care. Ensures that the VA medical facility has a dementia education plan for addressing all facility staff needs for education and training on the care of veterans with dementia. Primary contact for referrals to Geriatrics and Rehabilitation Service. Reviews and gathers all medical information for referrals from both internal and external sources. Facilitates the daily referral huddle to determine which referrals can and cannot be accepted for admission. Attends acute care huddles daily and each Veteran Village treatment team weekly to facilitate expeditious discharge/admission process. Is a member of several committees and work groups. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Education:
Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED) Licensure:
Must be licensed or certified by a state at the advanced practice level and must be able to provide supervision for licensure. (COPY OF LICENSE REQUIRED) GS-12 Grade Determination:
One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
