Senior Social Worker-HUD-VASH (Champaign/Decatur)

The HUD-VASH senior social worker is located at the Danville VA Medical Center and provides services in Champaign, Decatur, and surrounding communities. The HUD-VASH program is an intensive case management program for chronically homeless Veteran's and the most vulnerable Veteran's who have a history of homelessness and have complex heath care needs. The senior social worker will help the most vulnerable Veteran's benefit from timely outreach and meaningful engagement through assessment, service coordination, and support services to link Veteran's to permanent supportive housing in HUD-VASH or through community resources, while enhancing access to VA health care and benefits. The HUD-VASH senior social worker will coordinate with other specialty programs and services as needed for care management of chronic, complex heath care conditions among the aging homeless population. The senior social worker is responsible for verifying the Veteran's homeless status, disability status, eligibility for VA healthcare and VA Homeless programs, and service coordination needs for clinical stability. Major Duties:
- Provides case management for a caseload of Veteran's in the HUD-VASH program, providing clinical case consultation on high-risk Veteran's with colleagues in the VA and community based programs, and coordinating and prioritizing admissions to permanent supportive housing programs in the VA and in the community. - Responsible for reviewing lease agreements between the local Public Housing Authority, landlords, property management groups, and the Veteran. - Performs visits with HUD-VASH participants in the home, community, and health care settings to ensure Veteran's reside in safe environments with necessary supports to enhance independent living. Help ensure Veteran's are provided adequate support in meeting compliance with local housing codes and PHA guidelines, and advocating for emergency transfers in accordance with Violence Against Women Act (VAWA) when the Veteran and/or household members are attempting to flee domestic violence, dating violence, or stalking. - Responsible for ensuring housing and clinical stability is reassessed, critical time interventions are planned, and treatment planning is updated. - Actively participates in the monthly VISN 12 and National HUD-VASH Program conference calls and attends trainings offered by the National Homeless Program calls. - Responsible for following local policy and SOPs for the HUD-VASH program. This includes following policies and documentation requirements as required for CARF and Joint Commission accreditation. Will assist in developing or modifying SOPs as indicated for community or site specific needs. - Performs accountability functions including but not limited to:
Verifying Veteran's status, eligibility for VA health Care and VA Homeless programming service coordination needs; Documents all supportive services and case management contacts with Veteran/families/ancillary supports in CPRS; Administrative support for program coordination, development and implementation of written procedures specific to the community and surrounding areas; and Completes reports in accordance with VA Illiana HCS, professional discipline policies, and accrediting bodies (i.e., CARF, TJC). - Conducts psychosocial assessments to determine needs of Veteran's and family members - Actively participates in the treatment planning process with other disciplines in the VA and community based health care setting - Provides individual, family and group counseling as appropriate - Address Advance Directives in accordance with the standards of the work area - Will maintain a valid, un-restricted drivers license in order to transport Veteran's in government vehicles and provide a copy to the supervisor and will comply with all VA regulations regarding the use of government vehicles and transportation of Veteran's including driving physicals and submission of driving abstracts every year according to CARF requirements. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Education:
Must have a Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED) Licensure:
Must be licensed or certified by a state at the advanced practice level. (COPY OF LICENSE REQUIRED) GS-12 Grade Determination:
One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
